📋 Description

• Make home visits to clients in Forsyth County, NC.
• Evaluate functional status, muscle function, endurance, coordination, and ADL/IADL.
• Assess cognitive, psychological, motor aspects; develop intervention plans.
• Evaluate home hazards and environmental adaptations for independence.
• Identify equipment needs and revise care plans with nursing.
• Educate patients and families; document care using EMR/tablet.

🎯 Requirements

• NC Occupational Therapist license current.
• Minimum 1 year OT experience; home care preferred but not required.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
