Occupational Therapist, OT - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate functional status, including ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects; develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Continually revise the OT care plan with the multidisciplinary team.

🎯 Requirements

• Current South Carolina OT license.
• Minimum one year of recent OT experience.
• Accredited OT program per AMA/AOTA with transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
