📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, vision, coordination, ADL/IADL.
• Assess cognitive, psychological, and motor performance; develop interventions.
• Evaluate home environment for hazards to independence.
• Identify equipment needs and environmental adaptations.
• Document observations and interventions using EMR/tablet; revise care plan with nursing.

🎯 Requirements

• Current OH Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum one year of OT experience.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
