Occupational Therapist- Visits

📋 Description

• Evaluate patients’ functional status, including ADL/IADL and communication.
• Assess cognitive, psychological, and motor performance; craft individualized plans.
• Evaluate home environments for hazards and accessibility.
• Identify equipment needs and environmental adaptations.
• Revise OT care plan and coordinate with nursing on multidisciplinary plan.
• Educate patients and families in rehabilitative care for safety and independence.
• Document observations and interventions using a state-of-the-art touch pad tablet.

🎯 Requirements

• Be licensed as an Occupational Therapist in Hawaii.
• Must be willing to travel within the assigned territory.
• Graduated from OT curriculum accredited by AMA and AOTA.
• 1 year OT experience preferred.
