Make outpatient visits to clients in designated senior living communities.
Perform mobility tests and safety assessments (ROM, balance, gait, strength).
Develop and implement individualized care plans (manual therapy, gait/balance training).
Regularly reassess and revise the PT plan of care; participate in care plan meetings.
Educate clients, families, and staff to promote health, safety and independence.
Document observations, interventions, and evaluations using a touch-screen device.

🎯 Requirements

Massachusetts Physical Therapist license.
