Outpatient Occupational Therapist

📋 Description

• Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior
• Work closely with home health therapy partners to appropriately transition care from home health to
• Educate clients, families, and community caregivers on care plans to achieve a team approach to
• Professionally operate at top of licensure utilizing current research-based approaches to care
• Perform evaluations, treatments, and proper discharge planning using the outpatient EMR platform

🎯 Requirements

• Current VA Occupational Therapist license
• Prior home care experience preferred, but not required
• Minimum of one year of recent work experience as an Occupational Therapist
• Graduation from an occupational therapy program accredited by the Committee on Allied Health
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
