📋 Description

• Perform outpatient therapy evaluations to develop plans of care.
• Deliver care using evidence-based approaches to maximize independence.
• Collaborate with home health partners to transition to outpatient services.
• Educate client, family, and caregivers on care plans for teamwork.
• Document evaluations and plans using outpatient EMR.

🎯 Requirements

• Current North Carolina OT license.
• Minimum of one year OT experience.
• Graduation from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
