Outpatient Occupational Therapist, (OT) - Home Health

📋 Description

• Perform outpatient therapy evaluations to develop a care plan for adults in designated senior living communities or in a group home setting.
• Operate at top of licensure using evidence-based approaches to maximize functional independence.
• Work closely with home health therapy partners to transition care from home health to outpatient services.
• Educate the client, family and community caregivers on the care plans to maximize independence.
• Perform evaluations, treatments and proper discharge planning using the outpatient EMR platform.

🎯 Requirements

• A current North Carolina Occupational Therapist license.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited by AMA and AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
