Outpatient Occupational Therapist, (OT) - Home Health

📋 Description

• Perform outpatient OT evaluations to plan care in senior living communities or group homes
• Use evidence-based approaches to maximize functional independence
• Coordinate with home health to transition care to outpatient services
• Educate clients, families, and caregivers on care plans for teamwork
• Document evaluations and treatments in outpatient EMR; plan discharge

🎯 Requirements

• Current North Carolina OT license
• Home care experience preferred but not required
• Minimum one year of OT experience
• Graduate from AMA/AOTA-accredited OT program
• Ability to work independently and manage time effectively
• Solid computer skills; EMR experience preferred
