Outpatient Occupational Therapist - Senior Living Visits

📋 Description

• Perform outpatient therapy evaluations to plan care for seniors in designated senior living communities or group homes.
• Deliver care at top of licensure using evidence-based approaches to maximize function.
• Coordinate with home health partners to transition care from home health to outpatient services.
• Educate clients, families, and caregivers on care plans to maximize independence.
• Perform evaluations, treatments, and discharge planning using outpatient EMR.

🎯 Requirements

• Current Maryland OT license.
• Prior home care experience preferred, but not required.
• Minimum one year of OT work experience.
• Graduation from an OT program accredited by AMA and AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
