Outpatient Physical Therapist

📋 Description

• Make outpatient visits to clients in designated territories.
• Perform tests: mobility, ROM, transfer status, balance.
• Develop and implement individualized care plans, including manual therapy and gait training.
• Assess and revise PT care plan with nursing in the care team.
• Educate clients and families on rehabilitative care to promote health and independence.
• Document observations with a touch pad tablet.

🎯 Requirements

• Current VA Physical Therapist (PT) license.
• Graduation from an APTA-approved PT program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
