Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, balance, strength, endurance.
• Develop and implement individualized care plans with manual therapy and gait training.
• Continually assess and revise the PT care plan and coordinate with nursing.
• Educate clients and families in rehabilitative care to promote health and independence.
• Accurately document observations, interventions, and evaluations using EMR.

🎯 Requirements

• A PA Physical Therapist - PT license.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
