Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, ROM, balance, ambulation).
• Develop and implement individualized care plans (manual therapy, gait training, balance).
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health and independence.
• Accurately document observations and interventions in an EMR.

🎯 Requirements

• A current DE Physical Therapist - PT license.
• A minimum of one year of PT experience.
• Graduation from an APTA/CAAHEA approved program; evidenced by transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
