📋 Description

On-call LPN visits for Berks County on weekends (every other weekend)
Deliver skilled nursing and rehab care to adult clients in home settings
Visits scheduled 8:30am–5:00pm on Saturdays and Sundays
Coordinate with a multi-disciplinary health care team
Travel to home visits as assigned within territory
Document care and observations in EMR on a tablet

🎯 Requirements

PA LPN license (current)
Minimum 1 year LPN experience
Graduation from accredited nursing program
Prior home care experience preferred, not required
Ability to work independently and manage time
Strong interpersonal and computer skills; EMR experience preferred
