📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continuously assess and revise the care plan with the multidisciplinary team.
• Educate patients and families on rehabilitative care to promote independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Current PA SLP license.
• ASHA CCC-SLP or eligible for ASHA exam.
• Equivalent education and awaiting ASHA cert exam.
• Accumulating supervised experience for certification.
• Solid computer skills; EMR experience preferred.
