Physical Therapist

📋 Description

• Provide home visits for skilled physical therapy within designated territories.
• Perform evaluations of mobility, strength, balance, gait, and safety.
• Develop individualized treatment plans based on patient needs and orders.
• Implement therapeutic exercise, gait, balance, and mobility training.
• Provide integumentary and wound prevention interventions when needed.
• Collaborate with nursing and interdisciplinary team to coordinate care.

🎯 Requirements

• Current New Jersey PT license.
• Minimum one year of clinical experience as a PT.
• Graduate of an accredited Physical Therapy program.
• Ability to work independently and manage time.
• Strong communication, organizational and problem-solving skills.
• EMR experience preferred.
