Physical Therapist

📋 Description

• Provide skilled physical therapy services to adult and geriatric patients in their homes.
• Complete comprehensive PT evaluations: joint mobility/ROM, muscle strength, balance, gait, transfers, home safety.
• Develop and implement individualized treatment plans to maximize function, mobility, independence.
• Provide therapeutic exercise, manual therapy, gait/balance training, and evidence-based interventions.
• Educate patients and caregivers on home exercise programs, fall prevention, safe mobility, and equipment use.
• Reassess progress and modify treatment plans as appropriate; document in EMR.

🎯 Requirements

• Current Physical Therapist license in the State of New Jersey.
• Minimum of one year of recent Physical Therapist experience.
• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent.
• Home Health experience preferred, but not required.
• Strong clinical assessment, communication, organizational, and critical thinking skills.
• Comfortable using electronic medical records (EMR).
