Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements: mobility/ROM, balance, gait, strength.
• Provide wound care with evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait training, and balance.
• Assess and revise PT care plans with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care for health, and safety.

🎯 Requirements

• VA Physical Therapist - PT license.
• Graduate from an APTA/CAAHEA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
