Physical Therapist

📋 Description

• Provide home visit physical therapy to adult and geriatric clients.
• Evaluate mobility, strength, balance, gait, endurance, transfers, safety, function.
• Develop individualized treatment plans based on physician orders.
• Deliver evidence-based interventions: therapeutic exercise, gait, balance, mobility.
• Document evaluations and progress in EMR via tablet.
• Educate patients and families to promote safety and independence.

🎯 Requirements

• Current New Jersey PT license.
• Minimum of one year of clinical PT experience.
• Graduate of an accredited Physical Therapy program.
• Ability to work independently and manage time.
• Strong communication, organizational, and problem-solving skills.
• EMR experience preferred.
