Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, strength, balance, and safety assessments.
• Provide wound care with evidence-based dressings and topical agents.
• Develop individualized care plans with manual therapy, gait and balance training.
• Reassess and revise PT care plans; collaborate with nursing.
• Educate clients and families on rehabilitation and safety for independence.
• Document observations and interventions using EMR tablet.

🎯 Requirements

• Current New Jersey PT license.
• Minimum of one year PT experience.
• Graduate from APTA/CAHEA-accredited PT program (transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
