Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests: mobility/ROM, transfer, balance, strength, endurance.
• Use wound care dressings and topical agents to prevent and repair wounds.
• Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary care plan.
• Educate clients and family on rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• Active NJ PT license.
• Minimum 1 year of PT experience.
• Graduation from APTA/CAAHEA/AMA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
