Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements as needed (mobility, ROM, balance, strength).
• Implement integumentary protective interventions to prevent wound formation and promote repair.
• Develop and implement individualized care plans, including manual therapy, gait, balance training.
• Continually assess and revise PT care plans with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care and safety for independent living.

🎯 Requirements

• A current NJ PT license.
• Minimum one year of PT experience.
• Graduate from APTA/CAAHEA approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
