Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Conduct mobility/ROM/strength and balance safety assessments.
• Provide wound care and integumentary interventions.
• Develop individualized PT care plans, including manual therapy and gait training.
• Continuously assess and revise care plans; coordinate with nursing and multidisciplinary team.
• Educate clients and families on rehabilitative care to promote health and independence.

🎯 Requirements

• A current Delaware Physical Therapist (PT) license.
• Graduation from an APTA-approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
