Physical Therapist

📋 Description

• Perform home visits in designated geographic territories
• Complete comprehensive physical therapy evaluations
• Assess mobility, strength, balance, gait, transfers, endurance, and safety risks
• Develop individualized treatment plans to improve function and independence
• Provide therapeutic exercise, gait training, balance training, and other evidence-based interventions
• Collaborate with nurses and other clinicians as part of a multidisciplinary care team

🎯 Requirements

• Active and current Physical Therapist license in NJ
• Home healthcare experience welcomed, not required
• Ability to travel to cases as assigned
• Calm, reassuring presence with strong listening and communication
• Deliver patient care with compassion, excellence, and reliability
