📋 Description

• See a client from hospital discharge to a safe, comfortable home life.
• Perform diagnostic tests and measurements (mobility, strength, balance, safety).
• Develop and implement individualized care plans, including manual therapy and gait training.
• Continually assess and revise the PT care plan with nursing in a multidisciplinary team.
• Implement wound care with evidence-based dressings and topical agents.

🎯 Requirements

• Active and current license to practice as a Physical Therapist (PA).
• Home health experience welcome; training provided.
• Ability to travel to cases as assigned.
• Calm, reassuring presence with strong listening and communication.
• Compassionate, reliable patient care.
