Physical Therapist

📋 Description

• Perform home visits in designated geographic territories.
• Complete comprehensive physical therapy evaluations.
• Assess mobility, strength, balance, gait, transfers, endurance, safety risks.
• Develop individualized treatment plans to improve function and independence.
• Provide therapeutic exercise, gait training, balance training, and other evidence-based interventions.
• Collaborate with nurses and other clinicians as part of a multidisciplinary care team.

🎯 Requirements

• Current NJ Physical Therapist (PT) license.
• Minimum 1 year PT experience preferred but not required.
• Graduation from an APTA- or ACHEA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient computer skills; EMR experience preferred.
