Physical Therapist

📋 Description

• Perform home visits in designated geographic territories
• Complete comprehensive physical therapy evaluations
• Assess mobility, strength, balance, gait, transfers, endurance, and safety risks
• Develop individualized treatment plans to improve function and independence
• Provide therapeutic exercise, gait and balance training, and other interventions
• Collaborate with nurses and clinicians as part of a multidisciplinary care team

🎯 Requirements

• A current NJ Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from an APTA-approved program (APTA/CAHEA) as evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with EMR preferred.
