Perform skilled PT visits across designated territories.
Complete comprehensive PT evaluations and ongoing reassessments.
Assess mobility, strength, balance, gait, transfers, endurance.
Develop and implement individualized, evidence-based treatment plans.
Provide therapeutic exercise, gait and balance training, manual therapy, and education.
Collaborate with nurses, physicians, and the care team to coordinate patient care.

🎯 Requirements

Current New Jersey PT license.
Graduate of an accredited PT program.
Home health experience preferred, but not required.
Strong communication, organizational, and clinical assessment skills.
Ability to work independently with a flexible schedule.
EMR experience preferred and reliable transportation.
