📋 Description

• Make home visits to clients in designated territories.
• Perform mobility/strength/ambulation assessments.
• Implement wound care and dressings to promote healing.
• Develop and update individualized care plans, including exercises.
• Continually assess and revise PT care plan with the care team.
• Educate clients and families on rehab and safety.

🎯 Requirements

• A current NJ Physical Therapist - PT license.
• At least 1 year of PT work experience.
• Graduate from APTA/AMA-approved program (transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem solving skills.
• Proficient with computers; EMR experience preferred.
