📋 Description

• Make home visits to clients in designated geographic territories.
• Perform tests: mobility, ROM, gait, balance, safety.
• Implement wound care using evidence-based dressings.
• Develop individualized PT care plans, including exercises and gait training.
• Continually assess and revise the PT care plan with nursing.
• Educate clients and families on rehabilitation and safety; document observations in EMR on a tablet.

🎯 Requirements

• Current New Jersey Physical Therapist license.
• Minimum one year of PT experience.
• Graduate from APTA-approved program or AMA-accredited school.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
