📋 Description

• Perform skilled PT visits in designated territories.
• Complete comprehensive PT evaluations and reassessments.
• Assess mobility, strength, balance, gait, and function.
• Develop individualized treatment plans with evidence-based interventions.
• Implement skin integrity interventions to prevent wounds.
• Collaborate with nurses and interdisciplinary team for coordinated care.

Qualifications

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduate of accredited PT program approved by APTA or equivalent.
• Ability to work independently and manage time.
• Strong communication, organizational, and clinical assessment skills.
• Experience with EMR preferred.
