📋 Description

• Conduct skilled PT visits in designated territories.
• Perform comprehensive PT evaluations and ongoing reassessments.
• Assess mobility, strength, balance, gait, transfers, and endurance.
• Develop and implement individualized, evidence-based treatment plans.
• Collaborate with nurses and interdisciplinary team for care.
• Educate clients and families on exercises, safety, and mobility.

🎯 Requirements

• Current New Jersey PT license.
• Minimum of 1 year PT experience.
• Graduate of an APTA-accredited PT program.
• Ability to work independently and manage time.
• Strong communication, organizational, and clinical assessment skills.
• EMR experience preferred.
