Physical Therapist

📋 Description

• Perform skilled physical therapy visits throughout designated territories.
• Complete PT evaluations and ongoing reassessments.
• Assess mobility, strength, balance, gait, transfers, endurance, ROM.
• Develop individualized treatment plans using evidence-based interventions.
• Collaborate with nurses and interdisciplinary team to coordinate care.
• Complete timely documentation in BAYADA EMR.

🎯 Requirements

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduate of an accredited Physical Therapy program approved by the APTA or equivalent.
• Ability to work independently and effectively manage time.
• Strong communication, organizational, and clinical assessment skills.
• Experience with electronic medical records (EMR) preferred.
