Physical Therapist

📋 Description

• Provide skilled physical therapy services to adult and geriatric patients in their homes
• Complete comprehensive physical therapy evaluations, including assessments of joint mobility and
• Develop and implement individualized treatment plans designed to maximize function, mobility, and
• Provide therapeutic exercise, manual therapy, gait training, balance training, and other
• Implement appropriate interventions to promote skin integrity and support wound prevention and
• Collaborate with nursing and the interdisciplinary care team to coordinate patient care

🎯 Requirements

• Current Physical Therapist (PT) license in the State of New Jersey
• Minimum of one year of recent Physical Therapist experience
• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy
• Home Health experience preferred, but not required
• Strong clinical assessment, communication, organizational, and critical thinking skills
• Comfortable using electronic medical records (EMR)
