Physical Therapist

📋 Description

• Provide skilled physical therapy services to adult and geriatric patients in their homes
• Complete comprehensive physical therapy evaluations, including assessments of joint mobility
• Develop and implement individualized treatment plans to maximize function, mobility, and
• Provide therapeutic exercise, manual therapy, gait training, and balance training
• Implement interventions to promote skin integrity and wound prevention
• Collaborate with nursing and interdisciplinary care team

🎯 Requirements

• Current Physical Therapist license in the State of New Jersey
• Minimum one year of recent Physical Therapist experience
• Graduate of an accredited Physical Therapy program approved by APTA or equivalent
• Home Health experience preferred
• Strong clinical assessment, communication, organizational, and critical thinking skills
• Comfortable using electronic medical records (EMR)
