Physical Therapist- Full Time

📋 Description

• Conduct PT assessments: mobility, strength, balance.
• Develop and implement individualized care plans.
• Provide wound care using evidence-based dressings.
• Reassess and revise the PT care plan regularly.
• Educate clients and families on rehab activities.
• Document observations and interventions on a touch-pad tablet.

🎯 Requirements

• Current Hawaii PT license.
• Graduate from APTA/AMA-accredited program.
• Proficiency with Home Care Home Base (HCHB) and EMR.
• Strong computer skills; EMR experience preferred.
