Physical Therapist, Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostics and document findings (mobility/ROM, transfers, balance, strength, safety).
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement individualized care plans: manual therapy, gait training, balance.
• Assess and revise the PT care plan with the multidisciplinary team.
• Educate clients and families in rehabilitative care for healthy, independent living.

🎯 Requirements

• A current FL Physical Therapist license (PT).
• Minimum of one year PT experience.
• Degree from APTA/CAAHEA-approved program (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
