Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostics: mobility, ROM, transfers, strength, balance, safety.
• Implement integumentary protective interventions to prevent wounds using dressings.
• Develop and implement individualized care plans including exercises and gait training.
• Continually assess and revise PT care plans with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care for health and independence.

🎯 Requirements

• NH Physical Therapist-PT license.
• At least 1 year PT experience.
• Graduate from APTA/CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient with computers; EMR experience preferred.
