📋 Description

Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements: mobility, joint range, gait, strength, balance.
Implement wound care and protective interventions using evidence-based dressings.
Develop and adjust individualized care plans, including manual therapy and gait training.
Collaborate with nursing to integrate PT into multidisciplinary care plans.
Educate clients and families on rehabilitative care and safety for independent living.

🎯 Requirements

A current VA PT license.
Graduate from an APTA-approved program (transcript/diploma).
Ability to work independently and manage time.
Strong interpersonal, organizational and problem solving skills.
Solid computer skills; EMR experience preferred.
