Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Assess mobility, ROM, transfers, balance, and safety.
• Implement wound care using evidence-based dressings.
• Develop individualized care plans including manual therapy, gait, balance.
• Regularly assess and revise PT plan with nursing collaboration.
• Document care in EMR using a tablet.

🎯 Requirements

• Maryland PT license.
• At least 1 year PT experience.
• Graduate from an APTA/AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
