Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, transfers, balance, safety).
• Implement protective interventions to prevent wound formation and promote healing.
• Develop and implement individualized care plans, including manual therapy, gait training, balance.
• Continually assess and revise PT care plans, coordinating with nursing.
• Educate clients and family members in rehabilitative care to promote health and independence.

🎯 Requirements

• A Pennsylvania Physical Therapist license (current).
• Minimum 1 year of PT experience.
• Graduation from an APTA-approved program or AMA CAAHEA.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
