Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility/ROM tests, balance, gait, transfer status, strength, and safety assessments.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop individualized care plans, including manual therapy, gait, and balance training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care and safety for independence.

🎯 Requirements

• Maryland Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from an APTA- or CAAHEA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
