Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, transfers, balance, ambulation, strength.
• Wound care and protective interventions with evidence-based dressings.
• Develop and revise care plans with manual therapy, gait, balance.
• Educate clients and families on rehab, safety, independence.
• Document observations, interventions, and evaluations in EMR/tablet.

🎯 Requirements

• Maryland PT license required.
• Graduation from an APTA/CAAHEA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
