Physical Therapist - Home Health Visits

📋 Description

• Visit clients at home in designated territories.
• Assess mobility, ROM, gait, strength, balance, and safety.
• Provide wound care with evidence-based dressings and topical agents.
• Develop and adjust PT care plans with manual therapy and gait training.
• Collaborate with nursing to revise multidisciplinary care plans.
• Educate clients and families on rehabilitation, safety, and independence.

🎯 Requirements

• Maryland Physical Therapist license.
• Graduate from APTA-approved program or AMA COA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with EMR preferred.
