Physical Therapist, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements such as mobility, transfer status, balance, and endurance.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans including manual therapy, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan with nursing in multidisciplinary care planning.
• Educate clients and families on rehabilitation, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.

🎯 Requirements

• A PA Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduation from an American Physical Therapy Association (APTA)-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
