Physical Therapist, Home Health - Weekend Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, transfer, gait, strength, and balance tests.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and revise individualized care plans: manual therapy, gait, balance.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions using a tablet EMR.

🎯 Requirements

• Current CO Physical Therapist license.
• Minimum 1 year PT experience.
• Graduation from an APTA-approved program (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Travel throughout service area; EMR experience preferred.
