📋 Description

• Evaluate and assess patients' functional status and mobility
• Develop individualized treatment plans to maximize independence and safety
• Provide skilled therapeutic interventions in patients' homes
• Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other

🎯 Requirements

• Graduate of an accredited Doctor of Physical Therapy (DPT) program
• Licensed or license-eligible Physical Therapist in the state of practice
• New graduates are strongly encouraged to apply
• Excellent communication and interpersonal skills
• Valid driver's license and reliable transportation
