Physical Therapist, Per Diem Weekends, Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, gait, strength, endurance and balance assessments.
• Implement wound care and dressings per evidence-based protocols.
• Develop and adjust individualized care plans with manual therapy and gait training.
• Educate clients and families on rehabilitative care and safety.
• Accurately document observations, interventions, and evaluations on a touchscreen device.

🎯 Requirements

• Active and current PT license.
• One year of clinical experience preferred.
• Previous home health experience welcomed, but not required. We will train you!
• Ability to travel locally to cases as assigned.
