Physical Therapist, Per Diem Weekends, Home Health

📋 Description

• Visit clients at home to support recovery after injury, illness, or surgery.
• Provide one-on-one home health care to patients in their residences.
• Conduct mobility tests and ROM, gait, balance, and safety assessments.
• Implement wound care with evidence-based dressings.
• Develop and revise individualized PT care plans (manual therapy, gait, balance).
• Document care using a state-of-the-art tablet.

🎯 Requirements

• Active, current Physical Therapy license.
• One year clinical experience preferred.
• Home health experience welcomed; training provided.
• Ability to travel locally to cases as assigned.
