Visit clients in their homes in designated geographic areas
Perform diagnostic tests and measures (mobility, ROM, transfer, balance, gait)
Implement wound care and protective interventions with evidence-based dressings
Develop and revise individualized PT care plans, including manual therapy, gait training, balance
Document observations and interventions using a state-of-the-art touch pad tablet
Collaborate with nursing and the interdisciplinary team for comprehensive care

🎯 Requirements

Active and current PT license
One year of clinical experience preferred
Home health care experience welcomed, but not required (training provided)
Ability to travel locally to cases as assigned
